Insurance Information
At Endodontic Associates of Southern Indiana we make every effort to provide you with the finest surgical care and the most convenient financial options. To accomplish this we work hand in hand with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff. Please bring your insurance information with you to the consultation so that we can expedite reimbursement.
Dental Insurance
As a courtesy to you, we will bill your insurance company for both in network and out of network benefits. Please keep us informed of any changes to your insurance plan. Most insurance companies respond to the claim in 4 – 6 weeks. Any remaining cost after settlement of insurance claims is your responsibility.
We currently operate as an “in-network provider” with Aetna, Anthem, and Delta Dental. Once you schedule an appointment we will check your benefits and give you an estimated co-payment amount. Please note that until you come in for the treatment and it is settled with your insurance company this amount is not guaranteed.
If we are not “in-network,” you in many cases still have insurance benefits. We will gladly file an insurance claim on your behalf and will direct that any payment go directly to you. As we are unable to estimate those benefits, we require payment at the time of service and will promptly refund you any payment we receive.
Please note that you will most likely receive your Explanation of Benefits (EOB) before we do, if there is any overpayment we will mail you a refund check as soon as we receive the payment.
